Promotional Request Form

Giving back to our community is a tradition and a pleasure! Genesis Hair & Day Spa has contributed products, services, and monetary donations for years to the DeForest-Windsor community and surrounding area.

We receive numerous donation and promotional requests every year. Because of this, we will only consider requests that are submitted through this form with at least 30 days notice of your deadline. Your organization must be located within 20 miles of Genesis Hair & Day Spa.

We will direct all in-person and phone solicitations to this form for consideration. Please wait up to 5 business days for a response from us. If we decide to not contribute to your initiative, please understand that we can't contribute to every request. Thank you!
